The doctors welcome new patients who live within our practice area. If you move out of the practice area you may need to register with another practice.

All new patients are invited to make an appointment to have a health check with our practice nurse.

Register With Us

To register with us, please check you live within our catchment area; you can do this using the map below.

Once you have ensured you live within our boundary, please complete the online ‘New Patient Registration’ form.


This form should take 10 minutes to complete and it will take about 2 working days to get you registered. If at any point you find this form too hard, you can visit the practice to register a different way.

It cuts our administrative workload and makes it easier for you to register.

Registration forms are also available from Reception at the Practice, or are able to be downloaded here and brought into the practice once completed. Your name, address, postcode, date of birth and, equally important, the name and address of your previous GP, and your address whilst registered with that doctor, will be required. The latter information is vital to enable us to obtain your medical records. The form also has a space to enter your telephone number, town and country of birth, NHS number and previous surname, if you had one. We will need proof of ID and address if registering using the GMS1 form. Also ID and proof of address will be required if you are requesting access to medical records online or via any of the apps.


  • It can be an advantage for all the members of one family/household to be registered with the same practice and it is worth bearing this in mind when you decide to join a particular practice.
  • When you register you will be given a Surgery Leaflet 2024 containing some useful information about the day-to-day running of the surgery. You will also be given Form Audit C to complete. This is a short questionnaire, which screens for Alcohol, and all GP practices are now encouraged to obtain this information as part of a Government initiative.
  • Also included in the New Patient Pack will be a consent form for the text messaging service and an invitation to join our Patient Participation Group.

Catchment Area

We ask all new patients to book an appointment with one of our nursing staff for a general health assessment. This should take place within 3 months of registration, ideally earlier.

The main purpose of this appointment, which normally takes 20 minutes, is to gather information about your medical history. This includes medication that you may be taking, vaccinations you have had or may require, any allergies you may suffer from, and any important medical aspects of your family history. There is also the opportunity to check your blood pressure, urine and take height and weight measurements. On some occasions, if indicated, the nurse will also take a blood sample.

Following the assessment by the Health Care Assistant/Nurse, some patients will be advised to book an appointment with one of the doctors.
All patients at Flixton Road Medical Centre have a named, accountable doctor who is responsible for coordinating their care.
Your named doctor will be allocated to you by the practice. You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP.

If you have a preference and would like to request a particular doctor at the practice to be your named GP please talk to one of our receptionists.

– Fill in the Register with a GP surgery paper form and take it to the surgery you want to join.

– Contact the surgery you want to join directly.

Overseas Visitors

Those who visit the UK from overseas must pay for their care when they are in England.

In 2015 changes were made to how the NHS charges overseas visitors for healthcare so the NHS does not lose out on income for the services these patients have received.

Find Your NHS Number

Sometimes when filling in online forms or speaking with our staff, you may be asked for your NHS number.

This online tool will help you find your number if you do not know it.

Please Note

You do not need to know your NHS number to use NHS services, but it can be useful to have it.

Other ways to get your NHS number

If you cannot get your NHS number online you can:

  • find it on any letter from the NHS like a prescription or appointment letter
  • call your GP surgery and ask them for your number
