It can be an advantage for all the members of one family/household to be registered with the same practice and it is worth bearing this in mind when you decide to join a particular practice.
Paper forms are still available if you need one. When you register via the paper copy the receptionist will give you Form GMS1 to complete. Your name, address, postcode, date of birth and, equally important, the name and address of your previous GP, and your address whilst registered with that doctor, will be required. The latter information is vital to enable us to obtain your medical records. The form also has a space to enter your telephone number, town and country of birth, NHS number and previous surname, if you had one. We will need proof of ID and address if registering using the GMS1 form. Also ID and proof of address will be required if you are requesting access to medical records online or via any of the apps.
When you register you will be given a Surgery Leaflet 2024 containing some useful information about the day-to-day running of the surgery. You will also be given form Audit C to complete. This is a short questionnaire, which screens for Alcohol, and all GP practices are now encouraged to obtain this information as part of a Government initiative.
Also included in the New Patient Pack will be a consent form for the text messaging service and an invitation to join our Patient Participation Group.
Finally, we ask all new patients to book an appointment with one of our nursing staff for a general health assessment. This should take place within 3 months of registration, ideally earlier.
The main purpose of this appointment, which normally takes 20 minutes, is to gather information about your medical history. This includes medication that you may be taking, vaccinations you have had or may require, any allergies you may suffer from, and any important medical aspects of your family history. There is also the opportunity to check your blood pressure, urine and take height and weight measurements. On some occasions, if indicated, the nurse will also take a blood sample.
Following the assessment by the Health Care Assistant/Nurse, some patients will be advised to book an appointment with one of the doctors.
